FailSafe QI is a digital pocket guide to patient safety at UT Southwestern. It helps users understand event reporting, root cause analysis, streamlines submission of adverse events and near misses, and promotes participation in a patient safety culture. It supports reporting across Clements University Hospital, Parkland, Scottish Rite, and VA hospitals and clinics. The app provides quick access to educational videos on quality improvement methodologies and tools. It is available on both iOS and Android and is developed by the UT Southwestern Office of Quality Safety and Outcomes Education.
